Membership Application Form
Membership of MHNBF is open to qualified nurses who trained at The Middlesex Hospital and qualified nurses who served on the staff.
To apply for membership to the Middlesex Hospital Nurses Benevolent Fund, please complete the form here.
When your application form has been received, you will be contacted by the Membership Secretary, either by e-mail or by post, with details for payment of the subscription.
The Annual Subscription for new members, due 01 March each year, is £10.00. Overseas new members £15.00. This should preferably be paid by Standing Order (Mandate form will be sent). If paying by Sterling cheque, please make it payable to The Middlesex Hospital Nurses’ Benevolent Fund.
Do not send any money until requested.
Please note: MHNBF is a paper based organisation. The annual Journal is sent through the post.
Data Protection Policy: Personal details of members are maintained on an electronic data base for administrative purposes.
From time to time MHNBF may communicate with you by email through Mailchimp. In order to do this, we need your agreement.
